If you provide information about another person, you represent that you have permission or legal authority to provide the information and request that we contact you or that individual.
Career-Application Information
When you apply for an employment opportunity, we may collect:

  • Current area of residence;

  • Availability and preferred work area;

  • Employment and education history;

  • Professional licenses, certifications, and qualifications;

  • Languages spoken;

  • Résumé and supporting documents;

If additional screening is required, any background check, reference check, credential verification, or employment-eligibility process will be conducted with the notices and authorizations required by law.

Please do not submit the following through a general contact or career form unless the form specifically requests it and has been approved by One Stop for that purpose:

  • Social Security numbers;

  • Medicaid or insurance identification numbers;

  • Financial-account or payment-card information;

  • Complete medical records;

  • Detailed diagnoses or treatment information;

  • Government identification documents;

  • Account passwords; or

  • Other highly sensitive information.

If sensitive information is required, an authorized One Stop representative may direct you to an approved secure process.
Do not use the Site or its forms for emergencies. If someone may be experiencing a medical emergency, call 911 immediately.
4. How We Use Information
We may use information to:

  • Respond to questions and consultation requests;

  • Contact individuals and authorized representatives;

  • Discuss home care options and possible next steps;

  • Help individuals understand the Medicaid and MLTC process;

  • Coordinate with healthcare providers, plans, referral sources, or representatives when authorized or legally permitted;

  • Process and evaluate career applications;

  • Discuss suitable offices, cases, or work locations with applicants;

  • Provide language assistance;

  • Operate and improve the Site;

  • Maintain authorized workforce resources;

  • Protect accounts, systems, patients, applicants, and workforce members;

  • Detect fraud, misuse, security incidents, or unauthorized access;

  • Maintain required business and compliance records;

  • Fulfill legal, regulatory, licensing, and contractual obligations; and

  • Carry out another purpose disclosed when the information is collected or authorized by you.

Submitting information does not guarantee Medicaid eligibility, MLTC enrollment, home care authorization, caregiver availability, employment, or any specific outcome.

We may disclose information in the following circumstances:
Service Providers
We may use vendors that provide:

  • Website hosting and technical support;

  • Online forms;

  • Email, telephone, or text-message services;

  • Document storage and management;

  • Applicant and credential management;

These providers are permitted to access information only as necessary to perform services for One Stop and are expected to protect it appropriately. When a vendor creates, receives, maintains, or transmits PHI on our behalf, we will address applicable HIPAA requirements, including a Business Associate Agreement when required.
Care and Service Coordination
With appropriate permission or as otherwise allowed by law, we may disclose information to:

  • MLTC or insurance plans;

  • Healthcare providers;

  • Government healthcare programs;

  • Case managers or social workers;

  • Authorized family members or representatives;

  • Referral partners; and

  • Organizations involved in arranging or providing requested services.

Legal and Safety Purposes
We may disclose information when reasonably necessary to:

  • Comply with a law, regulation, subpoena, court order, or government request;

  • Meet healthcare, employment, licensing, audit, or reporting obligations;

  • Protect the rights, safety, and property of One Stop or another person;

  • Investigate fraud, abuse, security incidents, or unlawful activity; or

  • Establish, exercise, or defend a legal claim.

Business Changes
Information may be reviewed or transferred as part of a merger, acquisition, financing, reorganization, sale, or transfer of some or all of One Stop’s operations, subject to applicable confidentiality and legal requirements.
6. Sale of Personal Information and Marketing
One Stop does not sell personal information for monetary consideration.
We do not disclose information submitted through care-inquiry, contact, or career forms to third parties for their own independent direct marketing.
Mobile telephone numbers, text-message consent, and opt-in information will not be sold or shared with third parties or affiliates for their own marketing or promotional purposes. We may provide information to vendors that help deliver communications on our behalf, subject to appropriate restrictions.

You may ask us to stop nonessential marketing communications by using the unsubscribe instructions provided in the message or contacting us. Opting out of promotional communications does not prevent necessary administrative, service-related, security, or employment communications.
Email and ordinary text messaging may not always be secure. Please avoid sending sensitive medical or personal information through an unsecured method.
9. Protected Health Information
Some information One Stop receives may qualify as PHI under HIPAA. When information is PHI, its use and disclosure are governed by HIPAA, applicable New York law, and One Stop’s Notice of Privacy Practices.
The Notice of Privacy Practices explains rights that may include:

  • Requesting access to or a copy of health records;

  • Requesting an amendment;

  • Requesting certain restrictions;

  • Requesting confidential communications;

  • Receiving an accounting of certain disclosures; and

  • Filing a privacy complaint without retaliation.

This website Privacy Policy is not a HIPAA authorization. If this policy conflicts with the Notice of Privacy Practices or applicable law regarding PHI, the Notice of Privacy Practices and applicable law will control.
10. Information Security
One Stop uses reasonable administrative, technical, and physical safeguards designed to protect information against unauthorized access, use, alteration, loss, or disclosure.
These measures may include access controls, workforce training, vendor review, secure storage, authentication, system monitoring, and procedures for responding to suspected security incidents.
No website, email system, or electronic transmission can be guaranteed to be completely secure. You are responsible for protecting passwords and promptly reporting suspected unauthorized account access.
If a data incident requires notification, One Stop will provide notice as required by applicable law.
11. Information Retention
We retain information for as long as reasonably necessary to:

Retention periods vary according to the information’s purpose and applicable requirements. We may retain certain records after a request for deletion when required or permitted by law.

13. Career Applicants
Information submitted for employment purposes will be used to evaluate qualifications, communicate about opportunities, verify information, comply with employment laws, and maintain recruiting records.
Application information may be reviewed by authorized managers, human-resources personnel, credentialing personnel, and service providers involved in recruiting or onboarding.
Submitting an application does not guarantee an interview, employment, assignment, location, schedule, patient, case, or continued consideration.
14. Children’s Privacy
The Site is intended for adults and is not designed to solicit personal information directly from children under 13.
A parent, legal guardian, or authorized representative may contact us regarding services for a minor when applicable. If you believe a child has submitted personal information without appropriate permission, please contact us so we can review the matter.
15. Third-Party Websites and Platforms
The Site may link to platforms or websites operated by other organizations, including SharePoint, HHAeXchange, Slack, KakaoTalk, Adobe, healthcare plans, referral partners, and government agencies.
These third parties have their own privacy notices, terms, and security practices. This Privacy Policy does not govern information collected directly by an independent third-party platform.
Please review the third party’s privacy information before submitting personal or sensitive information.

We may update this Privacy Policy periodically to reflect changes in our Site, services, technology, legal obligations, or privacy practices.
